I had this same problem and it was caused by bad driver installation. To fix it here is what you should try:
Plugin your controller
Click start
Click control panel
(under hardware and sound in blue letters) click view devices and printers
VERIFY THE UNKNOWN DEVICE IS YOUR CONTROLLER FIRST BY UNPLUGGING IT FROM YOUR COMPUTER AND SEEING THAT IT DISAPPEARS OFF THE LIST
(plug controller back in to continue)
(If there is an unknown device) right click it and click properties
Click the hardware tab
Click the properties button
Click the change settings button
click update driver
Click browse my computer for driver software
click let me pick from a list of device drivers on my computer
scroll down until you see Microsoft common controllers for windows class
double click it
(On the list in the box that open select:) if you are using a wired controller: xbox 360 controller for windows version 2.1.0.1349 [8/13/2009]...if you are using a wireless receiver and controller: xbox 360 wireless receiver for windows version 2.1.0.1349 [8/13/2009]
click next
(in the new box that opens) click yes.
